 Ambulatory Care Centre


The Ambulatory Care Centre provides inter-disciplinary rehabilitation services and support to enable clients who are disabled, frail, chronically ill or recovering from surgery or injury, to achieve and retain optimal functional independence.

The team comprises nurses, social worker, physiotherapists, occupational therapist, speech pathologist, dietitians, podiatrist and a consulting geriatrician.

Services Provided

Please note that clients are responsibile for their own transport to and from sessions.

Services may also include assessment for home modifications.

How much does it cost?
  • $8.00 per session for health care card holder & low income earners, and
  • $12.40 per sesson for medium income earners
  • $78.70 per session for high income earners
  • $11.00 per Hydrotherapy session

The charges according to income range will be discussed with clients as part of the assessment process.

Additional costs may be incurred for specialised equipment or aids.  If you have difficulties, please discuss your situation with the Centre coordinator

How to access the service

You do not need a doctor's referral to access our services. However, a doctor's referral may be required after our assessment for acceptance to our programs. Requests for rehabilitation can be made to the Centre Coordinator by a doctor, the client, carers, family and/or other health and aged care services, both hospital and community based.

The team or an individual therapist, will assess all clients referred to the centre and prepare a treatment plan, together with the client and carer/s. The team will then regularly review the client and the client's GP is informed of progress and discharge.

Discharge from the program is coordinated with other services and community groups to promote ongoing maintenance of gains achieved and social re-integration where necessary.
