Diabetes Education
Seymour Health provides a qualified Diabetes Education Program by a Registered Nurse who sees clients by appointment. Clinics are conducted at the Ambulatory Care Centre opposite the Hospital. Access is via Villers Street. Appointments are available each Wednesday between 8.30am - 5pm and can be made by contacting ACC Reception on 5735 8050. A referral from your GP is required.
Clients with both Type 1 and Type 2 diabetes can access:
- Education
- Information
- Support
- Training in the use of Glucometers
- Provision of and training in the use of Novopens
- Dietary advice
- Referral to Dietitian and Podiatrist
What to bring to your appointment:
Medications list, including insulin
Copies of recent blood tests (your GP can help you with this):
- HbA1C
- Lipids
- LFT's
- Microalbuminuria
How much does it cost?
$8.50 per session for health care card holder & low income earners, and for all other clients irrespective of the number of health professionals seen.

The Hospital does not offer a free sharps container disposal service. This service is available from Mitchell Shire through the Seymour Library located in Anzac Avenue, Seymour.
