Chronic Disease Management - Better Care of Older People

 Hospital Admissions Risk Program (HARP) Team

      

The HARP BCOP Program is a community and centre-based service for people with defined chronic disease which is funded by DHS. 

We work together with clients to empower them to better manage their conditions.

Our services are part of Seymour Health

Who is eligible?
Residents from the Lower Hume, aged 65 or older with one or more of the following:

  • Chronic Heart Disease
  • Chronic Respiratory Disease
  • Diabetes

Who are we?

HARP-BCOP has a care coordination team with expertise in respiratory, cardiac and diabetes.  The service includes access to nursing, occupational therapy, physiotherapy and other health disciplines based on individual assessed need.

We work closely with other members of the multi-disciplinary health care team, including GPs and facilitate onward referrals as necessary.

AIMS

The aim of the HARP-BCOP service is to:

  • prevent emergency health crisis
  • assist clients to improve skills to reduce preventable deterioration
  • assist clients in maintaining their health
  • assist clients to have improved ability to manage their conditions
  • provide professional suppport in both the home and centre environments
  • reduce hospital admission and length of any hospital stay
  • facilitate maintenance groups for appropriate clients
How do I get on the Program?

Initially referrals are accepted from health professionals at Seymour Hospital inpatient wards, community services and Emergency Department.

How much does it cost?

Coordination of care by HARP-BCOP is at no charge to the client.  Equipment loan charges may apply.

Equipment

A range of equipment is available through the service for a short trial in your home.  If you decide to purchase any equipment, we can provide you with some options of places to purchase.

Privacy

We recognise that the sensitive information we obtain from you needs to be treated with respect and confidentiality.  The information we send onto your GP and/or other service providers will be done only with your consent.
