Palliative Care

The Lower Hume Palliative Care Service is based in Seymour (located in the Community Services building at the rear of the hospital) and provides care and support for clients with life-limiting illnesses, their families and carers.

The service can be provided to clients wherever the client chooses to be – home or hospital and services the clients living in the Shires of Mitchell, Murrindindi and the western area of Strathbogie.

The Palliative Care team comprises two Clinical Nurse Specialists, a Grief and Loss Worker / Coordinator of Volunteers and trained volunteers throughout the area. 


Do I need a referral?
No, you do not need a doctor’s referral, but we will require the patient’s consent to provide this service. After initial contact, we will do an assessment of the patient and his/her needs. We will then begin providing the agreed service from that moment with a referral to the District Nursing Service and/or a visiting volunteer if desired.  The service may continue for 14 months after the death of the patient.

What does the service include?
This is a holistic service and we will liaise with your doctor, hospital and other health care providers as required, to provide you with the best possible care and support of your choice throughout your illness, and support for your family/carer. We also have an equipment pool where you can borrow equipment to help you remain in your place of choice.

How much does it cost?
The service is free of charge.
