How to Access Services

You do not need a doctor's referral to access our services. However, a doctor's referral may be required after our assessment for acceptance to our programs. Requests for rehabilitation can be made to the Centre Coordinator by a doctor, the client, carers, family and/or other health and aged care services, both hospital and community based.

The team or an individual therapist, will assess all clients referred to the centre and prepare a treatment plan, together with the client and carer/s. The team will then regularly review the client and the client's GP is informed of progress and discharge.

Discharge from the program is coordinated with other services and community groups to promote ongoing maintenance of gains achieved and social re-integration where necessary.

How much does it cost?

Our fee is charged according to the Department of Health & Human Services Home and Community Care (HACC) guidelines for each person enrolled in the program.  The fee is discussed at your initial assessment appointment.  Additional costs may be incurred for specialised equipment, orthosis or aids / equipment.

The charges according to income range will be discussed with clients as part of the assessment process.
